Underwriting Checklist
for Performance/Payment, & Bid Bonds
- Contractors Questionnaire: this is a form describing your business and operations including banking information and references. We fax or e-mail send you the form upon your request.
- Company Financial Statements: Financial statements are required at fiscal year end and at the six month interim period. If available, please provide the last three years fiscal year end statements.
- Personal Financial statements: are required for each individual with 5% or more ownership in your company. Statements should be as of the same time as the companies fiscal year end statement.
- Work in Progress Schedule: is requested quarterly, stating all work on hand, original contract price, percentage of completion and change orders.
- Bid/Final (performance/payment) Bond Request: A copy of bid specs may be required. If you have a final (Performance/Payment) bond request, include a copy of the contract and bid spread information.
- Bank Letter: is a letter from you on your company's letterhead authorizing your bank to provide when your account was opened, if a line of credit exist, the availability of the line, any bank debt, etc.
- Resumes: resumes, if available on all owners and key personnel, these are especially useful if you have had bonded jobs, or if you are seeking a contract that is larger than you normally perform.
- General Indemnity Agreement: this document will require all owners and spouses to sign an indemnity agreement. This is standard for surety bonding. This document does not have to be completed until your bond has been approved, but must be completed prior to the bond being issued.
